Once we have received the completed form, we will contact your judicatory leader to confirm good standing, and arrange for you to authorize a criminal background check. Please note that particular service in a congregation is subject to an examination by the Committee on Ministry on matters of theology, faith and practice.<\/p>\n\n\n\n<p>If you have questions about
